Questions:

Is high temperature dangerous for the apical/root canal tissue?
According to many clinical tests temperature up to 200 degrees applied for few seconds does not have any undesirable effect on the tissue inside the root canal.

Why should gutta percha be condensed using cold pluggers?
Root canal obturaton should be three dimentional, tight and homogenous. For improving the sealing of thermal gutta percha and to minimize the risk air voids in the filling it is better to condensate the gutta percha with a cold plugger.

Why thermal gutta percha is considered the best way of root canal obturation?
Thermal gutta percha, correctly applied, gives the three dimentional root canal obturation. Together with tight apex sealing, as many clinical results show, it is the filling, which gives the least complications and the least re-endo treatments.

Root canal obturation step by step
1. Main gutta percha point fitting, it should fit tightly at working length (WL).
2. Choosing of cold pluggers (hand instruments)
3. Choosing a proper tip for a hot plugger.
4. After putting the main gutta percha point in the canal (with some sealer), the hot plugger should be switched on (it has the temperature of 200 degrees C) to place the point in the apical part of the canal.
5. The gutta percha in the apical part of the canal should be condensed with a cold plugger at first.
6. The hot plugger condensation - for about 2-3 seconds. Then the hot plugger should be switched off and held still for next 10 seconds to allow the gutta percha to cool down. The plugger should be once more switched on for 1 second to take it out of the canal.
7. The gutta percha in the apical and the middle part of the canal should be once more condensated with cold plugger.
Backfill
8. Unblock the pen.
9. Place the gutta percha bar in the pen device.
10. Wait for the pen to reach the temperature of 160 degrees Celsius (working temperature).
11. Hot thermoplastic gutta percha.
12. Fill the rest of the canal with hot thermoplastic gutta percha.
